چکیده
MAR\~N L. MEITUS, M.D., P.C.C.P. Miami Beach, Florida THE FIRST FETAL ELECTROCARDIOGRAM in utero was obtained by Cremer,' in 1906. Of the clinical applications of fetal electrocardiography described in the literature, the following are included: 1. Confirmation of fetal life in utero. 2. Detection of early (first-trimester) pre.pancy. 3. Proof of multiple pregnancy. 4. Dia~pmis of fetal position. 5. Prenatal detection of congenital heart dixase. 6. Contint~ous monitor in^ of the fet.11 heart during labor. 7. Postmatorit) or fetal anoxia. 8. Dia,pmis of fetal arrhythmias. D i r d e n of the maternal heart beat during pregnancy may develop in the presence or absence of organic heart disease. Sinus tachycardia. parox!smal supraventricular tachycardia. atrial flutter and atrial fibrillation may precipitate severe heart failure. There have been instances of fetal distress and of intra-uterine death attributed to intrapartum hypoxii~ asociated with impairment of the circulation in parox\xmal tachycardia and arrhythmias.' Pre<pancies 'romplicated by rheumatic heart disease and paroxysmal or established atrial fibrillation have been analyzed.' The incidence of heart failure and emhnlic episodes was higher in the atrial fibrillation

Direct current cardioversion in a case of congenital atrial flutter.
The congenital type of atrial flutter has a poor prognosis when unresponsive to digoxin (Moller, Davachi, and Anderson, 1969). Recorded experience with cardioversion is limited but it has been suggested that this type of flutter is refractory to electric countershock (Rodriguez-Coronel, Sueblingvong, and Hastreiter, 1968). We report an infant with congenital heart defects and congenital atrial ...
متن کاملElectrogenic Cardioversion
Atrial fibrillation (AF) is the most common arrhythmia in clinical practice. Cardioversion for AF may be performed by either using direct current (DC) shock (electrical cardioversion) or using drugs (chemical cardioversion). Here we report a case of a patient with heart failure and AF, who reverted to the normal sinus rhythm on correction of hyperkalemia (electrogenic cardioversion). The patien...
متن کاملCASE REPORT ECG diagnosis of native heart ventricular tachycardia in a heterotopic heart transplant recipient
A case is reported of haemodynamic collapse in a 51 year old male heterotopic heart transplant recipient caused by native heart ventricular tachycardia. An accurate diagnosis was made by selective right and left sided electrocardiography. Synchronised electrical cardioversion of the native heart (200 J) resulted in restoration of sinus rhythm with prompt relief of symptoms and amelioration of t...
متن کاملSuccessful Radiofrequency Ablation of Atrial Flutter Causing Hemodynamic Instability in a Patient with Recent Myocardial Infarction
Atrial flutter (AFL) is a common arrhythmia which may decrease cardiac output and may cause embolic events. Direct current (DC) cardioversion, medical cardioversion and radiofrequency (RF) ablation are therapeutic options, but over all RF ablation therapy has the longest event free period. Although development of AFL after myocardial infarction is quite common it may spontaneously recover or re...
متن کاملSuccessful ablation of refractory neonatal atrial flutter
Case report The patient was a male infant born at 38 weeks’ gestation. Atrial flutter was diagnosed in utero at 35 weeks of gestation, at which time no hydrops fetalis was present and no treatment was commenced. At 6 hours of life, the infant developed tachycardia. The electrocardiogram (ECG) showed atrial flutter with 2:1 atrioventricular conduction (Figure 1). Initially the rhythm could be re...
